- Your practice or organization name

- Your answers to the quiz or chat questions, such as practice size or the services you’re interested in
- Whether you’d like to receive email updates from us
- Anything else you choose to type into a message field
Please don’t send patient information through this website. Our website forms are not designed to receive protected health information (PHI), and we ask that you never enter patient names, medical details or other health information into them.
Information collected automatically
Like most websites, our site and hosting provider automatically record basic technical information when you visit, such as your IP address, browser type, device type, the pages you view, and the date and time of your visit. This is used to keep the site secure and working properly.

Our software platform and patient data
This policy covers this website only. Patient health information handled through The Bridge and our remote monitoring services is protected under HIPAA and the agreements we sign with our healthcare clients, including business associate agreements. Patients should refer to their healthcare provider’s Notice of Privacy Practices for how their health information is used.
Children’s privacy
This website is intended for healthcare professionals and businesses. It is not directed to children, and we don’t knowingly collect personal information from children under 13. If you believe a child has sent us information, contact us and we’ll delete it.

Our site links to other websites, such as our scheduling tool, government resources and news sources. Their privacy practices are governed by their own policies, not this one.
